- هو تمزق في واحد أو أكثر من الأوتار الخاصة بالعضلات الأربع المكونة للكفة المدورة والتي توجد في الكتف. حيث إن إصابة الكفة المدورة يمكن ان تشمل أي نوع من أنواع التهيج، أو زيادة الاستخدام لهذه العضلات والأوتار، وهي من أكثر الحالات التي تؤثر على مفصل الكتف. إن أوتار الكفة المدورة وليست عضلاتها هي الاكثرعرضةً للتمزق، ومن العضلات الأربعة فإن وتر عضلة فوق الشوكة هو الأكثر عرضةً للتمزق، خصوصاً عند نقطة مروره أسفل الأخرم; حيث يحصل التمزق عادةً عند منطقة التقاء وتر العضلة برأس عظم العضد، عند نقطة الحديبة العضدية الكبرى. الكفّة مسؤولة عن تثبيت المفصل الحقاني العضدي والقيام بحركات إبعاد العضد ودورانه للداخل والخارج، لذلك فإنه عند حصول رض للكتف ربما ستضعف هذه الوظائف وتتأثر.كذلك بسبب اعتماد الفرد بشكل كبير على مفصل الكتف في الكثير من الانشطة، فإن هذا الاستعمال الزائد للعضلات قد يؤدي إلى تمزقات، معظمها سيكون في وتر عضلة فوق الشوكة. In severe cases surgery may be tried, however benefits of surgery are unclear as of 2019. Rotator cuff tears are common. Those over the age of 40 are most often affected.
